สำหรับผู้ปกครองของนักเรียนใน B PRO ACADEMY

ข้าพเจ้า (ชื่อผู้ปกครอง) ในฐานะผู้ปกครองของ (ชื่อเด็ก) ซึ่งสมัครเข้าเรียนที่ B PRO ACADEMY ได้อ่านและเข้าใจรายละเอียดเกี่ยวกับการเรียน การเข้าร่วมกิจกรรม ตลอดจนเงื่อนไขการใช้บริการของ Academy แล้ว และขอแสดงความยินยอมภายใต้ข้อตกลงต่อไปนี้:

I, (Parent's Name), as the parent/guardian of (Student's rName), who has enrolled at B PRO ACADEMY, have read and understood the details regarding the training, participation in activities, and service terms of the Academy. I hereby give my consent under the following terms:

ส่วนที่ 1: การให้ความยินยอมเข้าร่วมกิจกรรมของ Academy

Part 1: Consent to Participate in Academy Activities

1. การรับทราบความเสี่ยงของกีฬาบาสเกตบอล (Acknowledgment of Risks Associated with Basketball)

   ข้าพเจ้ารับทราบว่า กีฬาบาสเกตบอลเป็นกีฬาที่มีการเคลื่อนไหวสูง และอาจเกิดความเสี่ยงในการบาดเจ็บจากการฝึกซ้อม หรือการแข่งขัน แม้ว่า Academy จะมีมาตรการดูแลความปลอดภัยแล้วก็ตาม

   I acknowledge that basketball is a high-movement sport and may involve risks of injury during training or competition, even though the Academy has implemented safety measures.

2. ข้อกำหนดด้านสุขภาพของผู้เรียน (Health Requirements of the Student)

   ข้าพเจ้าตกลงจะแจ้งข้อมูลด้านสุขภาพของผู้เรียนที่อาจส่งผลต่อการเล่นกีฬา เช่น โรคประจำตัว อาการแพ้ หรือข้อจำกัดทางการแพทย์ ให้เจ้าหน้าที่ของ Academy ทราบล่วงหน้า

   I agree to inform the Academy staff in advance of any health conditions that may affect the student's ability to participate in sports, such as chronic illnesses, allergies, or medical limitations.

3. การปฐมพยาบาลเบื้องต้น และความรับผิดชอบค่าใช้จ่าย (First Aid and Responsibility for Medical Expenses)

   ข้าพเจ้ายินยอมให้เจ้าหน้าที่ของ Academy ดำเนินการปฐมพยาบาลเบื้องต้นตามความเหมาะสม และเข้าใจว่า ค่าใช้จ่ายใด ๆ จากการรักษาพยาบาลเพิ่มเติม ที่เกิดขึ้นจะเป็นความรับผิดชอบของข้าพเจ้าโดยตรง

   I consent to the Academy staff providing appropriate first aid and understand that any further medical expenses incurred will be my sole responsibility.

4. การสละสิทธิ์ในการฟ้องร้องระหว่างผู้ปกครอง (Parental Waiver of Claims)

   1. หากเกิดบาดเจ็บ อุบัติเหตุ หรือข้อพิพาทระหว่างเด็กที่เข้าร่วมกิจกรรม ผู้ปกครองของทั้งสองฝ่ายจะไม่ดำเนินคดีหรือเรียกร้องค่าเสียหายซึ่งกันและกัน

      If an injury, accident, or dispute occurs between participating children, the guardians of both parties agree not to pursue legal action or claim damages against one another.

   2. ข้าพเจ้าขอสละสิทธิ์ในการเรียกร้องค่าเสียหายจากผู้ปกครองคนอื่น ๆ เด็กคนอื่น ๆ หรือ Academy จากเหตุการณ์ที่เกิดขึ้นจากการเล่นกีฬาตามปกติ

      I waive the right to seek damages from other parents, other children, or the Academy for incidents arising from normal sports activities.

   3. หากเกิดความขัดแย้ง ผู้ปกครองแต่ละฝ่ายตกลงที่จะแก้ไขปัญหาด้วยสันติวิธี โดยไม่ดำเนินคดีทางกฎหมาย

      Should any dispute arise, all parties agree to settle the matter through amicable negotiation and refrain from pursuing legal action.

5. ข้อจำกัดความรับผิดชอบทางกฎหมายของ Academy (Liability Waiver of the Academy)

   ข้าพเจ้ายอมรับว่า Academy, โค้ช, พนักงาน และเจ้าหน้าที่ จะไม่รับผิดชอบต่อการบาดเจ็บหรือความเสียหายใด ๆ ที่อาจเกิดขึ้นจากการเข้าร่วมกิจกรรม เว้นแต่เกิดจากความประมาทเลินเล่ออย่างร้ายแรง

   I accept that the Academy, coaches, staff, and personnel shall not be held liable for any injury or damage arising from participation in activities, unless caused by gross negligence.

6. คำแนะนำเกี่ยวกับประกันอุบัติเหตุ (Recommendation Regarding Accident Insurance)

   Academy ขอแนะนำให้ผู้ปกครองพิจารณาทำประกันอุบัติเหตุให้กับบุตรหลาน เพื่อรองรับค่ารักษาพยาบาลที่อาจเกิดขึ้นระหว่างการเรียนหรือแข่งขัน

   The Academy recommends that parents consider obtaining accident insurance for their child to help cover any medical expenses that may arise during training or competitions.

7. เบอร์ติดต่อฉุกเฉิน (Emergency Contact Number)

   ข้าพเจ้ารับรองว่าเบอร์โทรศัพท์ที่ให้ไว้สามารถติดต่อได้ตลอดเวลา และจะอัปเดตข้อมูลให้ Academy ทราบหากมีการเปลี่ยนแปลง

   I confirm that the provided phone number can be always reached, and I will notify the Academy of any changes.

ส่วนที่ 2: เงื่อนไขการใช้บริการคอร์สเรียนของ B PRO ACADEMY

Part 2: Course Terms and Conditions - B PRO ACADEMY

1. เงื่อนไขทั่วไป (General Terms)

   1. เมื่อลงทะเบียนและชำระเงินแล้ว ไม่สามารถขอคืนเงินได้ทุกกรณี

      Once registration and payment are completed, refunds are not allowed under any circumstances.

   2. คอร์สที่สมัครแล้ว ไม่สามารถเปลี่ยนเป็นคอร์สอื่นได้

      Enrolled courses cannot be changed to another course.

   3. 1 คอร์สใช้ได้สำหรับผู้เรียน 1 คนเท่านั้น ไม่สามารถใช้ร่วมกับผู้อื่นได้

      Each course is valid for one student only and cannot be shared with others.

   4. คอร์สมีวันหมดอายุ โดยเริ่มนับจาก วันที่เรียนครั้งแรก

      Each course has an expiration date, which begins on the date of the first session.

2. เงื่อนไขเกี่ยวกับวันหมดอายุของคอร์ส (Course Expiry Policy)

   1. การขอขยายวันหมดอายุ (ต้องมีเอกสารรับรอง) Request for Course Expiry Extension (Documentation required)
      • บาดเจ็บหรือป่วย โดยต้องมีใบรับรองแพทย์

        Injury or illness, with a valid medical certificate

      • เดินทางไปเรียนต่างประเทศ โดยต้องมีเอกสารแสดงวันที่เรียนอย่างชัดเจน

        Studying abroad, with official documentation clearly indicating study dates

   2. กรณีคอร์สหมดอายุไปแล้ว (In Case of Expired Course)
      • สามารถต่ออายุคอร์สได้ โดยการซื้อคอร์สใหม่ และทาง Academy จะโอนจำนวนครั้งที่เหลือจากคอร์สเดิมไปรวมไว้ในคอร์สใหม่

        The course can be renewed by purchasing a new one. Remaining sessions from the expired course will be added to the new course.

      • การต่ออายุนี้ ทำได้เพียง 1 ครั้งต่อคอร์สเท่านั้น

        This renewal can only be done once per course.

   3. การขอพักคอร์สชั่วคราว (ระงับการนับวันหมดอายุ) Request for Temporary Course Hold (Pause on expiry countdown)
      • คอร์สยังไม่หมดอายุ

        The course has not yet expired

      • ผู้เรียน บาดเจ็บหรือป่วย และต้องใช้เวลารักษานาน

        The student is injured or ill and requires an extended recovery period

      • มีภารกิจจำเป็นอื่น ๆ ที่หลีกเลี่ยงไม่ได้ ซึ่งส่งผลกระทบต่อการมาเรียน โดยจะพิจารณาเป็นรายกรณี

        There are other essential and unavoidable commitments that affect class attendance. Each request will be reviewed on a case-by-case basis.

การยืนยันความยินยอม (Acknowledgement and Consent)

โดยการกดปุ่ม "ยอมรับ" ด้านล่างนี้ ข้าพเจ้ายืนยันว่าได้อ่านและเข้าใจเนื้อหาในเอกสารฉบับนี้อย่างครบสมบูรณ์ พร้อมทั้งยินยอมให้บุตรหลานเข้าร่วมกิจกรรมของสถาบัน และยอมรับเงื่อนไขการให้บริการทุกประการตามที่ระบไว้

By clicking the "Accept" button below, I hereby confirm that I have fully read and comprehended the contents of this document, consent to my child's participation in the activities organized by the Academy, and unconditionally accept all the stated terms and conditions.

เงื่อนไขการใช้บริการ

1. หากชำระแล้วไม่สามารถคืนเงินได้ทุกกรณี [cite: 70]

3. คอร์สที่สมัครไม่สามารถใช้ร่วมกันได้ 1 คอร์สต่อ 1 คนเท่านั้น [cite: 72]
4. คอร์สที่สมัคร มีวันหมดอายุ โดยนับตั้งแต่ครั้งแรกที่มาเรียน [cite: 73]

กรณีที่เกี่ยวข้องกับวันหมดอายุคอร์ส

1. การขยายวันหมดอายุ ตามเอกสารที่ได้รับ [cite: 75]
